Locate the 2-digit alphanumeric batch code, (e.g.O2) etched on the front of the jig template.
Click to print and complete the form. Please print clearly. Incomplete or unclear forms will be returned.
Locate and cut out the UPC barcode on the RTJ400 carton (A barcode has black bars and a 12-digit number).
Mail the completed form, along with the UPC code and original receipt (keep a copy) to: Leigh Industries Ltd. P.O. Box 357, Port Coquitlam, BC V3C 4K6 Canada.
Submissions must be postmarked no later than November 30, 2022. Please have sufficient postage for mailing to Canada.
